Advanced Healthcare Directive
It has come to our attention that we do not have an Advance Healthcare Directive in your chart.
Why is it Important?
This form lets you have a say about how you want to be treated if you get very sick! This way, those who care for you will not have to guess what you want if you are too sick to tell them yourself.
What if I Change My Mind?
This form can be updated at any time by filling out a new form. Tell those who care for you about your changes, and give the new form to your medical decision maker and doctor.
What Happens Without This Form On Record?
If you are too sick to make your own decisions, your doctors will turn to family, friends or a judge to make decisions for you. These people may not know what you want. It is also a difficult position for your loved one or friend to navigate, so it is best to support these people in your life by preparing the Advance Healthcare Directive form and placing the form in your medical chart.
